1. If you decide to reactivate your membership in the future (and we hope you do) membership rates in effect at the time of reactivation will be applicable as membership rates are subject to change.

2. If you would like to put your membership on hold instead of cancelling, you can submit a Membership Hold Request. If you are sure you would like to cancel , complete and submit the form below. This will serve as your 30 day written cancellation notice as required by your membership agreement. I understand that my notice must be given 30 days before my renewal date. I understand that if my cancelation was given after 30 days prior to my renewal date , I will be charged for the following month and will have access to the facility until the final date.Your membership will be cancelled 30 days from the submission of this form Any scheduled billing within that 30 days must process. Your membership will be terminated 30 days after your last billing date. All payments are non – refundable.

3. I understand that upon my cancellation I will be removed from the members only FB page and welcomed back anytime In the future upon membership re-enrollment.

4. Cancellation of memberships prior to the expiration of any specified commitment period requires an early termination fee of the difference between the membership you were on and the membership you should have been on, as outlined in your membership agreement.
